Test Approval Application for Supplemental Exam(s)

THIS IS ONLY FOR EC-6 ESL, EC-6 Bilingual, and Special Education certification students who must complete one or more of the following exams:

#154 - ESL supplemental exam

#164 - Bilingual supplemental exam

#190 - Bilingual Target Language Proficiency Test

Please complete the form below to apply for test approval to take your supplemental exam. You will be required to upload proof of a passing score 80% or score of 240 or higher from a digital online practice test for your designated exams. (ex. 240 Tutoring, Certify Teacher, Passage Preparations, etc.)

*Please ensure your evidence clearly shows: the title of the practice exam, your passing score, and your name or identifying information if possible
